UNDERSTANDING THE RESEARCH

The details matter.

SoftWave belongs to the broader field of extracorporeal shockwave therapy (ESWT). These studies examine different shockwave technologies and protocols; they are not all trials of the SoftWave device used at Cloud 9. Each article’s findings apply to the patients and methods studied.

Use these summaries as a starting point. Your history and examination guide whether a treatment is appropriate for you.

PLANTAR FASCIITIS / 200401

Electrohydraulic shockwaves for persistent heel pain

Ogden et al. · Journal of Bone and Joint Surgery American · 86(10):2216–2228

What the researchers found

A placebo-controlled study included patients whose plantar fasciitis had not responded to at least six months of nonoperative care. At three months, the active-treatment group had more completely successful outcomes than the placebo group.

What to keep in mind

The study used a high-energy electrohydraulic device and ankle-block anesthesia. Its protocol should not be assumed to match an office SoftWave session.

SHOULDER / 200302

Shockwave treatment for calcific shoulder tendinopathy

Gerdesmeyer et al. · JAMA · 290(19):2573–2580

What the researchers found

In a randomized trial of 144 patients, both active-treatment groups improved shoulder function more than sham treatment at six months. High-energy treatment produced greater improvement than low-energy treatment.

What to keep in mind

Participants had chronic calcific rotator-cuff tendinopathy and received a specific image-guided protocol followed by physical therapy. This is not evidence for every cause of shoulder pain.

KNEE / 201503

How treatment energy may affect knee outcomes

Kim et al. · Annals of Rehabilitation Medicine · 39(4):616–623

What the researchers found

This study compared medium-energy and low-energy shockwave treatment for knee osteoarthritis. The authors reported greater improvement in pain and function in the medium-energy group.

What to keep in mind

The comparison was between two energy levels. It does not establish that all shockwave devices or treatment schedules are interchangeable, or that arthritis is reversed.

LATERAL HIP PAIN / 200904

Comparing shockwaves, exercise and injection

Rompe et al. · American Journal of Sports Medicine · 37(10):1981–1990

What the researchers found

A study of 229 patients compared radial shockwave treatment, home training and corticosteroid injection for greater trochanteric pain syndrome. Injection performed best initially; shockwave treatment did better at four months. At fifteen months, both home training and shockwave treatment had better success rates than injection.

What to keep in mind

This study used radial treatment, which differs from electrohydraulic SoftWave technology. Its findings concern lateral hip pain, not every hip disorder.

PLANTAR FASCIITIS / 200205

Why study results and protocols matter

Buchbinder et al. · JAMA · 288(11):1364–1372

What the researchers found

In this randomized placebo-controlled trial of 166 patients, both groups improved, but ultrasound-guided shockwave treatment did not outperform placebo on the measured outcomes at follow-up.

What to keep in mind

This contrasting result is part of the evidence. Patient selection, equipment, dose and follow-up vary across trials, so a positive study is not a guarantee of an individual result.
